Congress OpeningOpening lecture of Lipedema Evolution 2026, which sets the tone of the congress: treating lipedema by understanding fat as an endocrine organ and recognizing the role of Brazilian scientific output in the disease.
Amato opens the congress with the story of Kocher, who won the 1909 Nobel Prize for thyroid surgery by removing the entire gland and ended up creating hypothyroidism: the greatest surgeon of his time had to learn to cut less. He draws the parallel with lipedema, whose fat is also an endocrine organ that produces leptin and adiponectin, so removing everything without understanding what is inflamed would repeat the mistake of 1909 — the goal is to remove what is necessary, not the maximum. He presents the burden of diagnostic delay: an average of 16.8 years between symptom onset and diagnosis, with 60% of patients living with symptoms for more than a decade and 36% for two decades. He notes that from 2025 to 2026 more than 200 articles were published, which would take nine months to read and were condensed into two days of congress, with progress in clinical, surgical, hormonal, immunological, psychological and genetic approaches. Ultrasound in Lipedema — The Radiologist's PerspectiveThe radiologist presents the ultrasound patterns of lipedema described in the LDHC classification and the group's most recent finding: involvement of the fascia and muscle, which she calls lipedematous sarcopenia.
The speaker describes how she arrived at these findings: seeing patients with pain, edema and bruising whose vascular tests came back normal, she began to recognize recurring patterns of fat deposition on ultrasound, later confirmed by biopsy. She explains the morphological difference between obesity, which grows by hypertrophy while preserving the septa, and lipedema, which combines hypertrophy and hyperplasia until the septa rupture; they stop being linear, become curvilinear and branched, reach the dermis and produce fat herniations and the final fibrotic appearance. She describes the four grades of the LDHC classification, from the initial inflammatory pattern with a nodule in the hypodermis to fibrosis with skin traction, and notes that the exam is performed with an ordinary in-office linear probe, preceded by measurement of hypodermal thickness using the Amato protocol. In the second part, prompted by complaints of loss of strength in young patients, she shows that the chronic inflammatory process thickens the muscle fascia, reduces oxygen delivery to the fiber, and leads to mitochondrial dysfunction, myosteatosis and sarcopenia, with quadriceps entrapment visible on dynamic examination. Ultrasound in Lipedema — The Vascular PerspectiveThe vascular surgeon reviews the tests that support the clinical diagnosis of lipedema and details the four-point ultrasound protocol for measuring subcutaneous thickness.
The speaker starts from the Brazilian lipedema consensus, published in the Jornal Vascular Brasileiro, to recall that the diagnosis is clinical — history, symmetrical fat accumulation sparing the feet and hands, pain, tenderness to touch and capillary fragility — and that tests serve to document it. He covers densitometry with body composition (DEXA), with diagnostic criteria published in 2022, the QuASiL symptom questionnaire, also published and open access, and computed tomography and magnetic resonance imaging, which use the same parameters as ultrasound but cost more. He gives a history of the physics of sound and the development of ultrasound up to pulsed Doppler and the arrival of the device in Brazil in the 1970s. He notes that lipedema affects about 12.3% of Brazilian women and that approximately 60% of them have associated varicose veins, which makes ultrasound routine in the vascular consultation. He shows the ultrasound differentiation between lipedema and lymphedema, the honeycomb appearance and epidermal thickening of lymphedema, and a 2023 German study that detected neovascularization from stage 3 onward. Contraception in LipedemaThe gynecologist discusses the choice of contraceptive method in women with lipedema based on her own study of 637 Brazilian women, and presents partial data from an ongoing study on hormone replacement therapy.
The speaker reviews the role of estrogen beyond secondary sexual characteristics: regulation of adipose tissue metabolism, energy expenditure and leptin expression, mediated by three receptor types — alpha, beta and GPER — whose imbalance favors inflammation, metabolic dysfunction and weight gain. She recalls the difference in distribution between the sexes, with women having 10% more early preadipocytes in the abdomen and 35% more adipose tissue in the femoral region, and argues that lipedema has a multifactorial and polygenic origin not explained by estrogen alone. She presents the cross-sectional observational questionnaire study of 637 Brazilian women on hormonal contraceptives: the most used methods were the oral pill, implant, injection and vaginal ring; many reported worsening, and the most frequent side effects were weight gain (ranging up to about 6 kilograms), swelling and edema, leg pain, mood changes and decreased libido. Hence the approach she adopts: avoid starting with the oral route, prefer a non-hormonal IUD, accept levonorgestrel IUDs when necessary, and do not immediately withdraw the contraceptive when the symptoms predated its use. On hormone replacement, she shows partial data from 136 patients indicating that most do not worsen and that about 95% reported improvement in mobility, sleep, ecchymosis, pain, fatigue, weight and swelling, with no improvement in leg volume — the most used regimen was transdermal estradiol with micronized progesterone. She concludes that lipedema does not contraindicate hormone replacement and cites Lindenberg's work on the effect of hormones on femoral lipoprotein lipase.

What Physicians Need to Know About Anabolic SteroidsA pharmacology lecture on anabolic-androgenic steroids: formal and common indications, mechanisms, doses, adverse effects and why there is no support for their routine use in lipedema.
The speaker argues that physicians need to know these drugs to care for a population that today seeks information from coaches, forums and bodybuilding books before coming to the office. He establishes that the only formal indication for anabolic steroids is hypogonadism — noting that prior steroid use is itself a cause of hypogonadism through testicular atrophy — and that andropause does not exist, since physiological testosterone values span a wide range and there is no benefit in chasing high numbers. Among common indications, he cites catabolic states: sarcopenia and frailty, osteoporosis, autoimmune and neurodegenerative diseases, HIV, major burns, cancer and the postoperative period. He shows the classic trial with 600 mg of testosterone enanthate weekly, in which untrained men on placebo lost fat-free mass, those who trained gained 1.9 kg, those on testosterone alone 3.2 kg and those on testosterone plus training 6 kg, and points out that the study lasted 20 weeks, the phase of morphological adaptation to training. He goes through the pharmacology: aromatization and 5-alpha reduction, genomic and non-genomic pathways, and modifications of the molecule — 17-alpha alkylation, responsible for hepatotoxicity regardless of route, and the 19-nor family, from which nandrolone and gestrinone derive. He comments on the drugs with some clinical evidence (oxandrolone, stanozolol, nandrolone, methenolone) and warns that stanozolol lowers HDL by about 20 points. He insists that supraphysiological use always comes with harm, that there is no safe use under medical supervision, and that the concept of supraphysiological is different for women: oxandrolone alone already is, with risk of acne, alopecia, hirsutism, dysphonia and clitoromegaly, plus a post-cycle androgen insufficiency that no one knows how to manage. He denies that androgens make weight loss easier in women, using polycystic ovary syndrome as the clinical model of androgen excess. On lipedema, he states that there is no formal recommendation, that gestrinone implants have no support — citing a study by Dr. Alexandre — and that the use of black-market drugs such as drostanolone is unacceptable. He concludes in favor of caution, individualized risk assessment, education and harm reduction, with discontinuation as the ultimate goal.

Hormone Replacement in Women with LipedemaThe endocrinologist details how to prescribe hormone replacement therapy for women with lipedema: route, dose, choice of progestogen and practical clinical cases.
The lecture starts from the pathophysiology: overexpression of estradiol beta receptors in adipose tissue, leading to uncontrolled hypertrophy, chronic tissue hypoxia, perivascular fibrosis and an altered local immune response; the role of FSH, whose receptors in adipose tissue stimulate lipoprotein lipase and block UCP1, increasing fat uptake and reducing energy expenditure; and the greater activity of tissue aromatase, amplified by insulin resistance and excess cortisol. She cites a prevalence of 12.3% of women, almost 8.8 million in Brazil. She argues that replacement brings clear benefits — control of 70% to 90% of hot flashes, a 28% reduction in fracture risk and cardiovascular benefit within the window of opportunity — and that lipedema does not contraindicate it but requires individualization. She recommends avoiding oral estradiol because of hepatic first-pass metabolism and thrombotic risk, already increased in these women (about 0.6% versus 0.4%), preferring the transdermal route, with the "start slow, go slow" rule and initial doses of 0.25 to 0.5 mg/day, and avoiding applying the gel or patch over areas of lipedema. She explains a sudden rise in estradiol as the cause of worsening through increased vascular endothelial growth factor and plasma extravasation. She compares progestogens: micronized progesterone as first choice, with an antimineralocorticoid effect and mast cell membrane stabilization, but with a hangover effect in 10% to 20% of women due to allopregnanolone; the vaginal route and dydrogesterone as alternatives without this effect; and drospirenone when contraception and control of perimenopausal hormonal fluctuation are needed. She comments on the shortage of patches on the market, when a matrix-type patch can be cut, and the bleeding that appears when tirzepatide reduces absorption of the oral progestogen. Understanding the Genetics of LipedemaA dense lecture on the polygenic basis of lipedema: why there is no single gene, which polymorphisms recur across studies and how this genetic reading changes management in the office.
The speaker begins by distinguishing mutation, rare and disease-causing, from polymorphism, common and tendency-generating, to explain why such a prevalent condition cannot come down to a single gene. He goes through the findings of recent years: elastin and the tissue's difficulty returning to its initial position; the aldo-keto reductase family, the first gene described in a family with lipedema, whose altered progesterone inactivation clinically explains the patient who got much worse on contraceptives; the Italian and American panels with PPAR gamma, PGC1-alpha, PIK3CA, the histone polymorphism that facilitates DNA expression, and studies that found thousands of genes differentially expressed between patients. He comments on the UK Biobank GWAS, with 18 polymorphisms and emphasis on VEGF and COBLL1, and on SLC12A2/fibrillin, linked to cell adhesion and to intestinal and vascular leakage. He describes the biopsies that showed more CD31 and less cadherin, zonulin and claudin, that is, loose tight junctions — the leaky gut and leaky vessel that open phase 1 of the disease, with extravasation, hypoxia and pain. He also brings up non-celiac gluten sensitivity in 61% of patients and the seven times higher frequency of HLA-DQ2/DQ8 haplotypes, the MTHFR polymorphism three times more frequent, with the 677 enzyme functioning at 27% and compromising methylation, detox and glutathione, and the trio he calls office genetics: COMT, MTHFR and FUT2, the latter linked to constipation and a poor gut flora due to lack of fucose. He argues that gluteofemoral fat depends on specific genes, that interleukin 6 explains the disproportionate inflammation and nickel allergy, and illustrates with the case of a 59-year-old man treated with a glitazone for 4 months, whose subcutaneous fat rose from 144 to 204 and visceral fat fell from 140 to 105, with glucose going from 184 to 117 and glycated hemoglobin from 7.3 to 6.5. He concludes that the target of treatment is the endothelium, not the fat, and that he orders genetic testing for all patients.

Gut, Lipedema and GlutenA lecture that shifts the focus of lipedema from adipose tissue to the gut, proposing the mast cell and food reactivity — especially to gluten — as triggers of the inflammation that produces the diseased fat.
Amato uses the metaphor of a fire to argue that inflamed fat is merely the smoke of lipedema, and points to the mast cell as the arsonist: when it degranulates, it releases histamine and heparin and sets off a local chain reaction. He starts from the observation that very different exclusion diets (FODMAP, ketogenic, anti-inflammatory) improve patients, and reports that the group's meta-analysis of the ketogenic diet showed reductions in weight, waist and hip and, above all, reduced pain. When comparing IgG panels, he found a paradox: patients with lipedema have more IgG-positive foods, yet lower total IgG, as if there were immune exhaustion. In the HLA survey, 61% of patients carry HLA-DQ2 and/or DQ8 — twice as frequent with DQ8 alone and eight times as frequent with both together — although, in an open database with a lipedema phenotypic profile, they have less diabetes, less thyroid disease and celiac disease close to the general population, as well as 51% lower cardiovascular risk, suggesting a shield role. He reinforces the inflammatory hypothesis with cases of asymmetric lipedema always associated with another inflammatory event in the same limb. In practice, he proposes the analogy of the glass and the stone: rule out celiac disease, order HLA-DQ2 and DQ8, an IgG panel and/or a 12-week elimination diet with reassessment by symptom questionnaire; he cites a patient with 78% improvement after removing Brazil nuts. He acknowledges that the data are still preliminary and call for more studies.

The panel opens by rejecting the aesthetic use of anabolic steroids in patients with lipedema: the relevant effect is muscle mass gain, the low body fat percentage comes from the caloric deficit, and androgen excess worsens insulin sensitivity precisely in a disease that already alters it; before that, specific resistance training (hamstrings) and the nutritional intervention would need to be checked. Gestrinone is harshly criticized — it is not standard treatment for endometriosis, it has not been studied via the subcutaneous route, and the hormonal implant is described as a financial bias of the prescriber, in contrast with oxandrolone, which has oral evidence. On progesterone, the discussion is that there is no formal contraindication in lipedema, but there is little literature and much clinical intolerance, which improves when the inflammatory and intestinal background is treated; the general recommendation is estradiol, reserving progesterone for persistent sleep or mental health complaints. Hormone replacement in a woman with endometriosis and lipedema is considered possible, with an effective progestin to control the endometriosis. On thrombosis, the studies cited show that the transdermal route (gel, patch, spray) does not increase risk, with a risk of 0.9, and the absolute contraindication due to prior thrombosis is being revisited. The existence of a lipedema genetic panel is denied, citing two commercial panels used in practice (74 and 277 genes). In non-celiac gluten sensitivity with HLA-DQ2/DQ8, cutting gluten out completely is advocated — 88% of those with the gene improve symptomatically — with the explanation that the dramatic reaction upon reintroduction reflects a rested immune system. For mast cell activation, the first line is H1 plus H2 blockade (loratadine and famotidine), with sodium cromoglycate as a stabilizer already published in lipedema. The session closes with early menarche and endocrine disruptors.

Supporting GLP-1 Use and Formulations That Help Treat Lymphedema (Florien Symposium)A biochemistry lecture on the estrogenic axis of the adipocyte in lipedema and on how GLP-1 and GIP analogues, combined with venoactive drugs and herbal medicines, can support the treatment of lipedema and associated lymphedema.
Okigami starts from the tissue pathophysiology: the hypertrophied lipedema adipocyte produces cytokines, increases aromatase activity and begins to express more beta-type than alpha-type estradiol receptor, which makes it more inflammatory and closes a vicious cycle of more local estradiol, more beta receptor, more inflammation, more fibrosis and more aromatase. The hypoxia resulting from this inflammation stimulates glucose uptake, worsens hypertrophy and triggers mitophagy, leaving the adipocyte with few mitochondria. Within this rationale, GLP-1 and GIP agonists act by inhibiting the AKT pathway and GLUT4 translocation, reducing glucose uptake and lipogenesis, with an anti-inflammatory effect on adipose tissue and induction of diuresis and natriuresis, which gives the patient a quick sensation of volume reduction; he cites tirzepatide as the most used drug today and warns, based on a BMJ review, of weight regain after discontinuation, as well as discussions about osteoporosis, pancreatitis and the thyroid. Since he currently sees no way to convert beta receptor into alpha, he proposes targeting intracrinology by inhibiting aromatase — recalling that the adrenal gland accounts for 70% of peripheral estradiol in premenopausal women and 90% in postmenopausal women — and recounts that about 35 years ago he developed an aromatase-inhibiting cream for cellulite, whose patent had already been filed by another researcher. In the practical part, he advocates treating lymphedema together with lipedema using venoactive drugs and herbal medicines: he prefers Ruscus aculeatus standardized for ruscogenin, combined with hesperidin and vitamin C, cites calcium dobesilate, horse chestnut, bromelain for the purplish spots, resveratrol for its anti-inflammatory action and its effect on aromatase, organic silicon and proline for the vascular and lymphatic wall, and discusses a study with Pinus pinaster extract that, in his view, measured an effect more on lymphedema than on lipedema. He concludes that, in his view, lipedema is an endocrine disorder caused by excess estrogenic effect, via the beta receptor or elevated aromatase, worsened by insulin resistance, inflammation and oxidative stress.

The Lymphatic System in LipedemaA review of experimental and clinical data showing the two-way relationship between the lymphatic system and adipose tissue, and why the lymphatic component cannot be neglected in lipedema.
Mauro Andrade places the growing interest in fat in the context of the obesity epidemic and shows that publications on obesity, lipedema and lymphedema have grown in parallel over the last three decades — lipedema is not a new disease, it has only begun to attract attention. He presents his group's experimental model, in which circumferential skin incision and ligation of the lymphatics of the mouse tail produce increased volume, inflammatory infiltrate and an increase in fat cells in the distal region, with elevation of all adipogenesis markers evaluated; blocking the inflammation, whether with corticosteroids or in nitric oxide synthase knockout mice, reduces both the inflammation and the expression of these markers. On the clinical side, he recalls that in case series from two European clinics nearly 90% of patients with lipedema were obese. He also presents his own study, submitted and unpublished, on the distribution of lymphatic vessels by body region, and data comparing controls, lipedema, lymphedema and lipohypertrophy: the number of podoplanin-labeled vessels is similar, but their spatial distribution changes, and VEGF-D falls in lymphedema, lipedema and obesity; skin thickness increases in all three conditions and the degree of fibrosis is lower in pure obesity. He shows that obesity impairs lymphatic function — in mice on a high-calorie diet, collector contractions become irregular and do not respond to the stimulus that simulates drainage — and that, clinically, the higher the body mass index, the worse the function on quantitative lymphoscintigraphy. In models with Prox1 deficiency, lymphatic dysfunction precedes and favors weight gain; in other words, lymph makes you fat. He concludes by recalling the increase in interstitial fluid already demonstrated in lipedema by histology and bioimpedance, the microaneurysms of the initial lymphatics seen on microlymphography, the larger-caliber lymphatics and the altered platelet factor 4, to argue that the lymphatic system cannot be neglected in lipedema.

Nadai proposes that much of the signs and symptoms of lipedema can be explained by a deficit in energy production and advocates urinary organic acid metabolomics as a tool that measures function, not bioavailability — blood tells how much there is, urine tells what the cell was able to use. He reviews the endosymbiotic origin of the mitochondrion, the alternation of functions between light and dark (ATP by day; heat, melatonin, fusion and fission at night) and the role of PGC-1 alpha as the trigger of mitochondrial biogenesis, stimulated by physical exercise, energy deprivation with increased AMPK, fasting and cold exposure; he also mentions astaxanthin and berberine for their link to Nrf2 and PGC-1 alpha, and infrared light as a line of research, making clear that there is no study of mitochondrial biogenesis by infrared light in lipedema. The case is a 39-year-old patient with poor sleep, fatigue, anxiety, lower-limb edema, gastrointestinal complaints, cramps, sugar cravings and headache at the end of the day, on a ketogenic diet and daily exercise, taking sibutramine and pantoprazole, with a resting heart rate of 102 and cardiopulmonary exercise testing showing little available oxygen. Tests showed hepatic steatosis, insulin resistance, low vitamin D, zinc and manganese, estrogen dominance, elevated nonspecific inflammatory markers and high serum serotonin suggesting dysbiosis. On metabolomics, elevated pyruvate and lactate pointed to a block in pyruvate entry into the mitochondrion, alanine indicated muscle protein breakdown for gluconeogenesis, elevated isocitrate, succinate, fumarate and malate showed a stalled Krebs cycle with coenzyme Q10 deficiency, and low beta-hydroxybutyrate indicated that fat was not being used as an energy source. Management included rifaximin, colchicine, continuation of tirzepatide, discontinuation of sibutramine and pantoprazole, astaxanthin and berberine at night, tributyrin, lactobacillus strains, digestive enzymes, sublingual NAD, coenzyme Q10 in a more absorbable form, B complex with alpha-lipoic acid, and exercise at the first threshold and at the respiratory compensation point, without load, with calisthenics and resistance bands. Within 90 days the complaints improved, most notably with the disappearance of the headaches, and the test results improved; the loss recorded on bioimpedance was attributed to muscle glycogen, not muscle.

Frontiers of Molecular Research in LipedemaPresentation, by the author herself, of a multiomic study that searches for molecular signatures of lipedema in adipose tissue and blood and tests machine learning models to predict the disease.
Karen Herbst breaks down the article she co-authored, a cross-sectional multiomic study of 72 women with lipedema and 49 controls that combined RNA-seq and proteomics of adipose tissue with serum measurements of cytokines, amino acids, glutathione and sphingolipids. Leptin increased with body mass index in all groups, but adiponectin did not fall as expected, and high-molecular-weight adiponectin only differed at stage 3 — a sign that adipose tissue function is already altered at stage 1, independently of obesity. Serum ceramides were persistently elevated, which raises the question of a drop in S1P, the molecule that maintains endothelial barrier integrity, and of more permeable vessels. RNA-seq showed increased oxidative phosphorylation and cellular respiration with suppression of inflammation and immune cell activation, in both thigh and abdomen; proteomics, covering 4,987 proteins from 12 samples, showed increased mitochondrial organization and carboxylic acid metabolism and decreased complement and coagulation cascades, with reduction of the initiation, amplification and effector phases of complement. In blood there was a decrease in interleukin-5 and FLT3L, an increase in VEGF, a decrease in glutamic acid at all stages and an increase in methionine sulfoxide, a marker of reactive oxygen species, as well as a downward trend in the reduced/oxidized glutathione ratio. The predictive model used random forest, support vector machine and elastic net with 81 lipid parameters and three adipokines from 121 participants; random forest performed best, with 11% false positives, still high, and the core signature consisted of ceramides, sphingolipids, lactosyl- and hexosylceramides, adiponectin, high-molecular-weight adiponectin and leptin. The interpretation she proposes is of a tissue that is metabolically hyperactive and oxidatively stressed, yet immunologically blunted, rather than classically inflamed fat: lipedema would be a chronic mitochondrial, vascular and adipocyte disorder with maladaptive immune suppression, consistent with pain, edema, fibrosis and capillary fragility without marked elevation of CRP or cytokines. Open questions remain as to why the vessels are permeable, whether there is extracellular matrix alteration, whether S1P is low and whether the signature can become a biomarker.

Interoceptive Dysmorphia in LipedemaA lecture proposing the concept of interoceptive dysmorphia to explain why the aesthetic complaint of patients with lipedema tracks their inflammatory symptoms rather than the volume of their legs.
Amato starts from the classic definition of body dysmorphia and cross-references two variables in the QuASiL questionnaires from his practice: leg volume, measured in liters, and the aesthetic complaint on a 0 to 10 scale. Cases with large volume and high complaint are consistent, but he finds patients with little volume and extremely high complaint and, on follow-up, patients whose volume fell and whose aesthetics worsened, and others whose volume increased and whose aesthetics improved — in other words, volume and aesthetics do not go together. To explain this, he uses the analogy of the recorded voice: the brain merges airborne sound with cranial vibration and produces a voice that only its owner knows. In front of the mirror, the woman adds to the visual image everything she feels in terms of pain, heaviness, inflammation, fatigue and swelling, while the world, including her husband, sees only the visual part. When the complaint improves with treatment of the inflammation alone, it is no longer merely a mental issue: it is interoceptive dysmorphia, a sensory channel the observer cannot see. In the data presented, symptoms correlate more with the aesthetic complaint than volume or weight do, accounting for almost half of the final result, and he describes 74 patients who gained an average of 3.5 kg in their legs and nevertheless came to see themselves positively, with a 26-point drop on the questionnaire. He observes that the thin, inflamed body is the one that suffers most and is most disbelieved, cites Penfield's homunculus and the comparison with a paronychia to show how inflammation hijacks the brain's attention, and reports the recalibration of patients who deny pain at the first visit and give a real score at follow-up. Among the symptoms, he mentions 90% reporting leg pain, 48% burning and 35% tenderness to touch. The practical recommendations are to believe the invisible symptom, ask before labeling, target the symptom rather than the scale, rank the complaints by priority, and be careful with surgery, because liposuction is effective at reducing pain, but pain is precisely the inflammation marker that signals the underlying problem.

Benitti opens with an Italian study published in Nutrients that analyzed the microbiota of women with lipedema and found 70% more intramuscular fat than in women without the disease, as well as an excess of bacteria and a scarcity of the good ones. He explains the balance between Firmicutes and Bacteroidetes, associating a predominance of Firmicutes with an inflammatory profile and weight gain, and recalls the flora transplant experiments that transfer the lean or obese phenotype. On small intestinal bacterial overgrowth, he states that the clinical history already identifies a good share of cases, that FUT2 alteration with chronic constipation is associated with the condition, and recommends testing for urinary indican, an inexpensive test, instead of the methane test, treating with rifaximin when positive. The chain he describes is: dysbiosis, inflammation and opening of the one-cell-thick intestinal barrier, entry of lipopolysaccharides and continuous immune activation, with leg fat serving as a receptacle by genetic determination, a decline in butyrate-producing bacteria and a vicious cycle that also explains 39% less lean mass than in other women, despite a muscle genetics he describes as excellent. He highlights the beta-glucuronidase of Firmicutes, which cleaves the estradiol conjugate in bile and causes it to be reabsorbed, linking lipedema, endometriosis, polycystic ovary, uterine fibroids and breast cancer to the same cycle; he says he is against the use of aromatase inhibitors, which he tried in 2018 and which worsened the joints. He presents microbiota signatures in other diseases — molecular mimicry between the bacterial wall and thyroid peroxidase in Hashimoto's, estradiol recirculation in endometriosis, and a predictive signature in Parkinson's — and describes office findings, not backed by a study: Clostridium innocuum inactivating progesterone in constipated patients, treated with metronidazole 100 mg for one to two weeks; Klebsiella as the largest histamine producer in patients with long-standing rhinitis and sinusitis; and Escherichia coli, which increases sugar cravings, inactivates progesterone, ascends via the vagus nerve and causes neuroinflammation. For treatment he cites lactoferrin instead of oral iron, pomegranate 250 mg as an E. coli antagonist, glutamine for the enterocyte, calcium D-glucarate 500 mg every 12 hours to block glucuronidase, quercetin, curcumin and epigallocatechin gallate in liposomal form, zinc carnosine 30 mg for the tight junctions, vitamins A and D, omega-3, the Mediterranean diet and specific strains such as Lactobacillus plantarum. He concludes that the target is not the fat, but the balance of the body as a whole.

Regenerative Medicine in LipedemaAn orthopedic surgeon presents the musculoskeletal differential diagnoses of lipedema and the biological rationale for regenerative therapies — orthobiologics and physical therapies — targeting the inflammatory and fibrotic pathways of the disease.
Mosaner starts from the orthopedist's view of the patient with lipedema: bilateral leg pain, hypersensitivity, edema that does not improve with compression or drainage and progressive functional limitation, usually without a diagnosis. He goes through the differential diagnoses by region — arthralgia, osteoarthritis, osteochondral lesion and synovitis in the ankle, Baker's cyst and pes anserine tendinitis in the medial knee, trochanteric bursitis in the hip — comments on the marble pattern of adipose tissue on ultrasound and on Stemmer's sign, and shows that the complaints also overlap, because medial knee fat increases the valgus moment and joint hypermobility favors sprains and instability. He cites a study in which about 35% of patients with lipedema also have a diagnosis of fibromyalgia, through a shared mechanism of peripheral and central sensitization. He argues that conventional therapies are not enough because they do not address chronic low-grade inflammation, septal fibrosis, microvascular and lymphatic dysfunction and pain sensitization, and that the desirable targets — M1 to M2 macrophage polarization, TGF-beta inhibition, lymphangiogenesis and nociceptive modulation — coincide with the mechanisms of regenerative therapies. He then reviews PRP, noting the lack of standardization and the shift from the concept of concentration to platelet dose, with about 8 billion platelets established for knee osteoarthritis; BMA and BMAC, citing the doctoral thesis of Fabio Lana, which compared the two in knee, hip and spine osteoarthritis with two years of follow-up and similar results, which would make concentration unnecessary; and adipose tissue products, SVF and microfat, with the caveat of studies indicating that SVF obtained from liposuctioned fat from an area with lipedema has increased endothelial permeability and would not be a good option for regenerative purposes. Among physical therapies, he presents photobiomodulation, which acts on mitochondrial cytochrome c oxidase, with a case series in lipedema describing apoptosis of hypertrophied adipocytes, inflammatory modulation and aromatase regulation, and shockwave therapy, aimed at septal fibrosis and sensitization, with evidence for reduction in thickness and in post-liposuction fibrosis. He places the final emphasis on preparing the soil, summarized in the acronym SODIMER — sleep, diet, medication and supplements, microbiome, exercise and rehabilitation — with targets of glycated hemoglobin below 5.4 and uric acid below 6 in men and below 5 in women. He illustrates with two cases: a chronic wound with an indication for amputation that closed in three months after two sessions of a membrane with BMA and L-PRF, and an O'Donoghue triad treated with BMA during the pandemic, with 80% symptomatic improvement in one month and a reconstituted ligament on MRI at a year and a half. He concludes that there is a biological rationale and indirect evidence, but direct clinical studies in lipedema are lacking, and that no specialty can solve the case alone.

Dib begins by acknowledging that there is still no robust evidence directly linking GLP-1 analogues to lipedema, but points to the indirect correlation: reduction of metabolic syndrome, total weight and visceral fat leads to lower secretion of pro-inflammatory cytokines such as interleukin 6 and TNF-alpha. His central argument is that the success of the medication should not be measured during use, but after it stops. He describes what he calls the suppression gap in three scenarios: the prior one, with very poor diet quality, bad habits, lack of sleep and high stress; the one during use, in which the person enters a programmed malnutrition, eats very little and poorly, loses water, muscle glycogen, muscle tissue and adipose tissue, and has no energy to train even though weight drops; and the ideal one, which is the one the person is able to sustain after stopping the medication, with controlled calorie intake, adequate protein intake, sugar control, training and hydration. He warns, as a personal conclusion and not as literature data, that significant weight regain will come in the coming years among those who stop the drug, as was already seen after bariatric surgery became widespread. On muscle mass, he cites the evidence that 25% of the weight lost with tirzepatide is muscle mass, but insists that there is no direct causal relationship between the drug and this loss: the correlation is the same as with bariatric surgery or extremely restrictive diets, and part of the reduction is expected and acceptable within total weight loss. He sums up that the drug silences hunger, but it is the omission of nutrition that consumes muscle, and advocates using the medication as a bridge to a new lifestyle, including in patients with lipedema, because of the overall improvement in habits. Genetics of Lipedema — UK Biobank GWASThe genetic epidemiologist presents the genome-wide association study of lipedema conducted in the UK Biobank, the replicated loci and the research path they open.
The speaker frames lipedema among complex traits: the heritability of body fat distribution is around 50%, and GWAS of waist-to-hip ratio and liver fat have shown that these traits are influenced by thousands of variants with very small effects, with no dominant gene. He notes that there are no twin or family studies for lipedema, but that the proportion of patients with a family history ranges from 16% to 64%, and ventures that heritability is at least 50%. Lacking diagnosed cases in sufficient numbers, the group used the UK Biobank, with 500,000 adults, and defined an approximate phenotype: women above the 65th percentile of leg fat percentage and below the 50th percentile of waist circumference, a cutoff chosen to produce a prevalence of 10%. The GWAS identified 18 loci; replicated in an independent cohort of about 130 clinically diagnosed cases were the GRB14/COBLL1 loci on chromosome 2 and VEGF-A on chromosome 6, with only nominal significance for LYPLAL1 and ADAMTS9. Pathway analyses pointed to EGFR1 and actin monomer signaling, suggesting variation in the remodeling of the stromal vascular tissue, with enrichment in arterial blood vessels. He also cites a recent preprint with a meta-analysis of two clinical cohorts, 128 cases from Spain and 148 from the United Kingdom, which did not reach significance on its own but supported three of these loci. As next steps, he advocates expanding case identification through diagnostic codes and imaging in biobanks, increasing sample sizes, including rare variants through whole-genome sequencing and investigating the biological mechanisms, which may point to therapeutic targets.

The panelists answer five questions. On electrical stimulation, local or whole-body, they consider that it increases adherence because it is a short workout, of about 20 minutes, and activates deep muscles that are hard to recruit in conventional training, but it should be an add-on and not a substitute. On the patient who arrives sarcopenic, it is argued that treating sarcopenia without investigating the cause, age of onset, biochemical markers, body composition and habits is simplistic, and the general rule proposed is 2 grams of protein per kilogram of body weight on an at least normocaloric diet, with resistance and load training. On adapting squats and leg press in lipedema, the recommendation is to assess joint mobility, start with small ranges of motion and work on myofascial release in the long term, and the preference is to schedule strength and cardiovascular training on different days so they do not compete. On surgical indication in the knee, the boundary between surgery and regenerative medicine is discussed based on the type of injury and the patient's profile, with the example of an athlete who avoided a knee replacement, and the caveat not to use blood-derived products in a highly inflamed patient. Finally, on prolonged GLP-1 use and muscle hypertrophy, the answer is that there is harm when the person cannot maintain the necessary protein and calorie intake, but that gains are possible if training and diet are adhered to.

Five papers are presented. The first is a prospective, double-blind, randomized, placebo-controlled trial of LED phototherapy in patients with mild to moderate lipedema: 20 patients and 40 legs, four 10-minute sessions with an LED blanket versus a placebo blanket, assessed with a visual analog scale and the SF-36 immediately, at 30 and at 90 days; both groups improved, with no statistically significant difference, and the conclusion was that the positive effects were not related to the phototherapy itself. The second is a case report of infrared thermography in the monitoring of an 81-year-old woman with stage 4 lipolymphedema, lymphorrhea and recurrent erysipelas, showing a reduction in hyper-radiant areas between the active infection phase and the controlled phase under complex decongestive physical therapy, ozone therapy, pneumatic compression and ILIB. The third, derived from a master's thesis, reports two cases of knee valgus and postural changes in women with stage 2 and 3 lipedema, describing the chain of internal femoral rotation, valgus and foot pronation, with improvement in alignment and pain after 12 weeks of clinical Pilates. The fourth describes a phenotype compatible with lipedema in a 29-year-old man with prior obesity who lost about 70 kilograms with diet and exercise alone and retained a gynoid distribution documented by densitometry, with an android/gynoid ratio of 0.75, CRP of 4.8, high homocysteine, vitamin D insufficiency and normal testosterone, improving with a ketogenic diet, vitamin D correction and resistance exercise. The fifth investigates histamine metabolism dysfunction in a 40-year-old patient who underwent four surgeries in just over a year and was still symptomatic, with altered fecal zonulin, reduced sIgA, IgG food reactivity, high homocysteine and elevated fecal and salivary histamine, treated with the five Rs protocol with resolution of pain. The session closes with an account of the role of support groups and regional associations, based on a survey of a group of more than 700 women in which the main complaint was lack of information and diagnostic delay, with 73% reporting prejudice and 59% reporting depression, culminating in the approval of the law on comprehensive care for patients with lipedema in Minas Gerais.

Lipedema Surgery Evidence ReviewA review of the recent scientific evidence on lipedema reduction surgery, including the first randomized clinical trial in the field.
Wright places surgery within a holistic approach: a chronic, progressive disease with no cure, in which non-surgical treatment — compression, anti-inflammatory diet, weight control, lymphatic therapy — is the consensus first line and must be fully implemented before surgery is considered. The surgical indications are persistent pain, swelling or tenderness, functional limitation of gait, mobility or knee mechanics, and progression despite conservative treatment. He advocates replacing the term liposuction with lipedema reduction surgery, which emphasizes that a disease is being treated, with a reconstructive goal, and makes clear that it reduces without curing. He cites the systematic reviews and meta-analyses by Mortada, Coutinho and Amato, with more than 20 studies and 14 prospective cohorts, showing improvement in quality of life, pain, pressure sensitivity, bruising, leg heaviness, walking difficulty and itching. The randomized LipLeg trial, funded by the German government, compared conservative treatment alone with conservative treatment plus surgery: 7.6% of patients reported a significant reduction in pain in the first group versus 68.3% in the second, an odds ratio of 26 to 1, with significant improvement defined as a drop of at least 2 points on a 1-to-10 scale. The gains in mobility and knee function are comparable to those of a total knee arthroplasty, and Baumgartner describes durability of up to 12 years. On safety, he discusses lymphatic protection (abundant tumescence, cannula parallel to the long axis of the limb), declines to define a safe aspirate volume by a single number — although volumes of 5 liters or more are sometimes necessary —, prefers to stage large volumes and recommends tranexamic acid to reduce bleeding. He concludes that variability in technique and indication is still large and should decrease with new studies.

A dermatologist with more than 30 years of treating lipedema, Rapprich begins with diagnosis: disproportionate pain that appears during phases of hormonal change, and edema — which he considers an accidental name, since it is not present in the early stages, making painful lipohyperplasia the more appropriate term. He demonstrates the thumb test to distinguish non-pitting tissue from pitting edema, and notes that the current German guideline does not recommend the three stages, because a stage 1 patient may have more pain than a stage 3 patient. Obesity is an independent comorbidity but worsens the condition: weight is treated first (exercise, diet or GLP-1 agonists, citing a patient with a 35-kilogram reduction) and only afterwards is the lipedema operated on, since it is resistant to weight loss. The whole leg is treated in three procedures — inner thigh, outer thigh with buttock and hip, and lower legs —, at least four weeks apart and with one week off work; young normal-weight stage 1 patients can be done in two. He details tumescence according to Jeff Klein, with lidocaine infiltrated at 700 mL per minute, deep down to the fascia, respecting 50 mg/kg and a red line of 12 liters, about 1 liter per 10 kg of body weight. He uses 4 mm, 35 cm vibrating cannulas with a Mercedes tip for all areas, in a windshield-wiper motion, leaving a final thickness of 2 cm; sedation with midazolam and ketamine, tranexamic acid (which he calls a game changer for reducing hematomas), and antibiotic and antithrombotic prophylaxis for 10 days. He prefers power-assisted liposuction to water-jet liposuction, considering it less aggressive — yellow aspirate, without blood. He also details the technique in the arms, with five needles and channels that are never repeated. Postoperatively he prescribes lymphatic drainage for three weeks and compression for as long as edema persists, and reports a recurrence rate of around 10%. He offers an online surgical atlas and an app for patients, both still in German.

How to Address Skin Laxity in LipedemaA lecture on preventing and treating skin laxity in lipedema surgery, from preoperative preparation to deep and superficial retraction technologies.
Thaís Torres Neves argues that preventing laxity begins long before the operating room. The tissue of a patient with lipedema is histologically altered, more congested and with more interstitial fluid, and bringing a patient to the table while still inflamed or infiltrated produces more fibrosis and, consequently, more irregular retraction. Laxity is not just excess skin: supporting tissue is lacking, and muscle is built preoperatively, with resistance exercise (weight training, free weights, water exercise, well-performed Pilates) and protein intake, cited as 2 grams per day. She shows the case of a patient who lost 27 kilograms in 6 months and arrived without supporting structure, with empty septa, to argue that rapid weight loss with GLP-1 analogues requires waiting for the tissue to settle — one year of conservative treatment, or more. The surgeon also causes laxity: thick, heavily perforated cannulas destroy septa, and inadequate infiltration causes bleeding and more fibrosis; she uses mechanical infiltration with large volumes and staged surgeries. She divides the technologies into deep retraction, which requires prior undermining — Renuvion (helium-gas radiofrequency, focal plasma, fast and controlled, good for the knee), Argoplasma (argon gas, dissipates heat, treats larger areas with more risk to neighboring structures), BodyTite (bipolar radiofrequency with an external thermal-control electrode, reaches subdermal planes and compacts fat) and Quantum (tip bipolar, without an external electrode, deeper and broader, with fewer studies) — and superficial retraction, with microneedling radiofrequency, above all Morpheus, whose brush function fires at three depths in a single shot and whose scale function protects the skin from heat. She uses the patient's own fat as support and filler, and biostimulators (poly-L-lactic acid and calcium hydroxylapatite) as maintenance after six months to one year. She closes by insisting on compression therapy before and after surgery — without compression there is seroma — and on the message that treating laxity means preserving and regenerating, not merely retracting skin.

Complex Surgical Cases in LipedemaA lecture on the surgical treatment of upper-limb lipedema in complex cases, advocating separating treatment into two stages — volume first, then skin.
Ana Carolina Nadai starts from a stated discomfort: the normalization of poor results, the idea that an ugly result is acceptable because the case was difficult. She notes that arm lipedema is present in 80% of affected patients and is often underdiagnosed, and that traditional staging only classifies the lower limb — which is why she proposed a classification for the upper limb, with types, patterns and staging. Before surgery, the patient needs to understand that she has a disease, that surgery does not cure it and that management is permanent. She presents the technique she developed, Lipo 360 (which she intends to rename Slicing), based on Langer's lines of tension, which are circular in the arm: circumferential undermining favors immediate centripetal accommodation of the skin, followed by retraction during healing. This represents more than 15 years of follow-up and 272 cases in the last three years, all without retraction technology, with the same team for 12 years. Unlike the previous speaker, she declares herself in favor of extensive undermining. She also describes a simple laxity grading, measuring in centimeters, with the arm at 90 degrees, how far the arc of skin sags from the line between the elbow and the axillary fold. The central argument is that traditional brachioplasty, described since 1930, was designed for the massive-weight-loss patient, with redundant skin and little volume; in the lipedema patient there is a binomial — volume and laxity — and trying to solve both in a single operation is the great mistake, a source of dehiscence, seroma and unsightly scarring. After twenty years of trial and error, she concludes that the best results come from dissociating: first debulking with liposuction, then, three months later, resecting the skin. She shows cases including the forearm (which she considers essential to master), results at 4, 15, 30 and 60 days, a patient who went from dress size 54 to 42, and insists on custom-made compression garments, a trained team and general anesthesia to ensure ventilation when repositioning large patients.

The two surgical speakers answer questions from the audience. On keloids in brachioplasty, it is argued that they result from skin tension: by debulking the arm first and dealing only with the redundant skin in the second stage, the scar closes practically without tension; liposuctioning and resecting in the same operation has the added problem of the infiltration, whose fluid drains through the scar, prolongs healing, leaves friable tissue and favors dehiscence — the only problem that staging does not solve is the anxiety of the patient and the surgeon. Whether the inflammation caused by the technologies can worsen lipedema is discussed: the answer is that an inflamed patient does not go to surgery, that the inflammation sought is controlled, that when in doubt surgery is postponed, and that these patients require more intensive postoperative monitoring and compression therapy. After Morpheus, no tape is applied and drainage is not allowed in the first week, so the skin can be monitored; after seven days the patient is cleared, always with the same trained physiotherapist, using gentle maneuvers — excessive manipulation increases the risk of fibrosis. The most interesting disagreement of the session emerges: one of them states that fibrosis is exactly what she wants, the key to retraction, while the other responds that, in the lower limb of a patient with lipedema, excessively deep fibrosis creates areas of depression and irregularity, with results that collapse after the first six months. Neither uses endolaser for lipedema; endolaser is restricted to the treatment of varicose veins, and combined saphenous and lipedema surgeries are avoided because of the greater tissue injury and inflammatory response. On grafting, the guidance is not to use fat from diseased areas — it is harvested from regions such as the proximal inner thigh —, in small volumes, to correct step-offs and support points, with fat increasingly understood as regenerative tissue.

Stem Cells, Inflammation and Adipose RemodelingA presentation of basic research on adipose-derived stem cells in lipedema, with data on inflammation, angiogenesis, extracellular matrix and hormonal regulation.
Sara Al-Ghadban, a researcher at the University of Virginia, presents her group's line of work on adipose-derived stem cells (ASCs) in the pathogenesis of lipedema. In the study published in 2019, comparing tissue from patients with lipedema to obese and non-obese controls, she found a significant increase in CD68-positive macrophages even in non-obese patients, suggesting that inflammation is an intrinsic trait of the tissue and not a consequence of obesity. In the thigh, she observed an increase in the number and caliber of vessels, with dermal vessels extending toward the epidermis, accompanied by greater macrophage infiltration; histology showed hypertrophic adipocytes, dilated vasculature (CD31 and SMA), fibrosis on trichrome staining and an infiltrate of CD68 macrophages and CD117 mast cells. Isolated from thigh and abdomen and confirmed by the markers CD73, CD90 and CD105, lipedema ASCs formed more colonies and maintained identity and similar proliferation rates, but showed greater adipogenic potential, with no change in osteogenic differentiation — a lineage bias. Gene expression revealed differences between depots and an increase in inflammatory and angiogenic markers such as IL-1 beta, IL-6 and VEGF, although variability limited significance in some comparisons. Differentiated adipocytes expressed more leptin in the thigh and more PPAR-gamma in the abdomen; 3D spheroids confirmed the findings, with more LPL and GLUT4. In the extracellular matrix there was a decrease in laminin and metalloproteinases with an increase in cytokines, an imbalance that favors fibrosis. Conditioned medium from lipedema ASCs and adipocytes increased CD31 and Ang-2 in endothelial cells, raised MMP9 and leptin and, in the Matrigel assay, produced significantly more tubes, confirming a pro-angiogenic effect. Estradiol raised alpha and beta receptors and PPAR-gamma, and M1 and M2 cytokines altered morphology and differentiation, showing the immune–adipose crosstalk. The conclusion is that lipedema would be a stem-cell-driven tissue remodeling disorder, not a simple accumulation of fat.

Biopsy of LDHC 3 NodulesA case report with ultrasound-guided excisional biopsy of subcutaneous nodules classified as LDHC 3 on ultrasound, with histopathological and Doppler analysis.
A vascular surgeon and sonographer, Anderson Nadiak Bueno investigates what the nodules seen on ultrasound in the LDHC (Lipedema Dermal Hypodermal Classification) actually are, making clear that the LDHC 3 nodule, subdermal and visible only on ultrasound, is not the palpable nodule described in clinical classifications. In a patient whose points of most intense pain coincided exactly with the nodules, he performed ultrasound-guided marking and excisional biopsy in the operating room under local anesthesia, on the posterior aspect of the right thigh and the right arm; he biopsied all four classifications, but only the nodules showed changes. When opened, the material looked like blood. Hematoxylin-eosin histopathology described an extensive area of hemorrhage, hemosiderin deposits indicating non-recent bleeding, fibrotic proliferation, a focus of fat necrosis with ghost cells, eosinophilic granular content, a histiocytic reaction and neoangiogenesis, without vasculitis. The Doppler study of the subcutaneous vessels showed a higher resistance index in patients with lipedema: 0.83 versus 0.68 in the thigh (a 22% increase) and 0.75 versus 0.66 in the arm (a 13% increase), which contradicts the expectation of low resistance in an inflammatory condition and raises the hypothesis of ischemia. The proposed pathophysiology links adipocyte hyperplasia and hypertrophy, subcutaneous compartment syndrome, immature and fragile neoangiogenesis, bleeding, fat necrosis, inflammatory recruitment with heparin and histamine, edema, and the closing of the cycle. He also describes four ultrasound patterns of nodule, all with the same histological findings, and previews a paper in press differentiating the LDHC nodule from lipoma by the disorganization of the surrounding fat. As an unpublished observation, the two biopsied patients reported pain improvement from 10 to 0 after the procedure. He concludes that clinical classifications are of little use, that ultrasound works as a virtual biopsy and that multicenter, multidisciplinary studies are lacking.

Tirzepatide in LipedemaA lecture examining the biological plausibility of using tirzepatide in lipedema, through the action of the GIP receptor on the adipocyte, and delimiting what is still a hypothesis.
Ana Paula Peclat poses the question of whether tirzepatide, beyond reducing weight, modifies the biological mechanisms of the fat cell in lipedema. She describes the disease through four interacting pillars — hypertrophy, chronic inflammation, progressive fibrosis and vascular and lymphatic microangiopathy — and characterizes the lipedema adipocyte as larger and metabolically less efficient, with insulin resistance, lower glucose uptake, greater expression of pro-inflammatory and pro-fibrotic genes and reduced levels of the protein that mobilizes stored energy, which means less thermogenesis: an adipocyte that stores and does not mobilize. Tirzepatide differs from the selective analogues in being a dual GLP-1 and GIP agonist, with formal indications for type 2 diabetes, obesity and sleep apnea, and results superior to GLP-1 analogues in weight loss, blood glucose and triglycerides. The central argument is topographic: the GLP-1 receptor acts mainly in the central nervous system, with little or no expression in the adipocyte, whereas the GIP receptor is highly expressed in adipose tissue, acting on lipolysis, fat-cell metabolism and insulin sensitivity. This effect depends on the metabolic context: after feeding, GIP increases insulin sensitivity, activates lipoprotein lipase and favors healthy storage; in the fasting state, or with the receptor chronically activated by the drug, it stimulates lipolysis, reduces lipogenesis, favors browning of the white cell and increases fatty acid oxidation. The mechanisms at both ends fit together, but the speaker closes by clearly delimiting the boundary: there are no randomized clinical trials in lipedema, no regulatory approval for this indication and no post-tirzepatide histological studies; there are only initial case reports. Tirzepatide is not yet a treatment for lipedema, but rather one of the most biologically coherent hypotheses to have emerged in recent years.

How Laser Can Enhance the Appearance of the Legs in LipedemaA presentation of a body and vascular laser protocol developed in partnership with industry, for lipedema patients who are already clinically compensated.
Igor Sincos presents a protocol developed about a year and a half ago in partnership with Fotona, combining vascular and body treatment in the same device. He makes clear from the start that it is not a protocol for women with severe inflammatory lipedema, but for those already transitioning to a stable clinical condition, seeking an aesthetic result beyond the functional one. The equipment combines two lasers: the Nd:YAG 1064, used both for reticular and varicose veins and, with a robotic body handpiece, to act deep in the fat, reaching up to 47 degrees when thermolysis is desired — a temperature he advises against in lipedema, because it can cause even more inflammation —, and the Erbium, superficial, aimed at skin quality and tightening, in non-ablative mode, without peeling. He describes four body handpieces in addition to the vascular one: the Erbium T-Runner, the Nd:YAG NX-Runner, vacuum, and the Marco handpiece, for low-energy photobiostimulation, the only one he considers suitable for inflamed lipedema. In the lipedema patient he begins with the Erbium, preparing the skin, and continues with the Nd:YAG in piano mode at a lower temperature, seeking biostimulation of collagen and elastic fibers rather than fat burning. He insists on rigorous candidate selection and a 360 diagnosis, with a vascular surgeon mandatory on the certified team, because he finds many diagnostic errors — patients treated as lipedema with saphenous reflux, post-thrombotic syndrome and even open wounds. He proposes five balances (nutritional, metabolic-hormonal, vascular, physical and emotional-spiritual) as eligibility criteria, and vascular and body planning in parallel. He illustrates with the case of a patient with varicose veins since adolescence and right great saphenous reflux, treated with saphenous ablation, three vascular sessions and four body sessions focused on the buttocks and saddlebags, with results assessed at 120 days; the biological effect, he warns, takes three to four months. He reinforces that technology is the icing on the cake on top of nutrition and physical activity.

He criticizes lymphoscintigraphy, which may be read as lymphedema when it is actually a functional change of lipedema, besides being time-consuming and invasive, and considers CT and MRI expensive and lacking a measurement protocol that produces a number for follow-up. He presents as experimental indocyanine green, photoacoustic lymphangiography, 3D scanning with the iPhone LiDAR camera, and thermography, the latter with a single publication, on the effect of compression stockings on temperature. He closes with the ranking of sensitivity and specificity — ultrasound and DEXA both with a ROC curve of 0.91 — and with the message that lipedema is not seen on imaging, it is confirmed by it: the diagnosis remains clinical and imaging serves to turn the symptom into a number.

Treatment of Varicose and Spider Veins in Patients with LipedemaHow to adapt the treatment of varicose veins and telangiectasias when the patient also has lipedema, discussing evidence, hybrid laser techniques and expectation alignment.
The speaker starts from the question of whether varicose veins can be treated equally in patients with and without lipedema. She describes the vascular terrain of lipedema — thickened, fibrotic vessels, increased endothelial permeability, intensified angiogenesis, perivascular fibrosis and capillary fragility — and shows the epidemiological overlap: 35.5% of patients with lipedema have varicose veins and venous insufficiency, 61% have spider veins and almost half have already had surgery, with foreign cohorts reaching 80% associated venous insufficiency. She reviews the scarce literature: a German study from 2000 with 261 patients operated on for varicose veins (68 with lymphedema, 103 with lipolymphedema, 90 with lipedema) showed that surgery did not improve edema, with 90% unchanged and fewer than 10% with relief of heaviness and fatigue; and a study published earlier this year in Phlebology, with 48 patients, pointed to paresthesia and smaller gains after thermal ablation in those with lipedema. She proposes a strategy in which the patient is first de-inflamed and treated clinically, and only then approached vascularly, with hybrid treatment, high-frequency ultrasound as a game changer for mapping depth and course, and expanded use of laser. She presents her personal algorithm, which she calls the laser sandwich, with successive passes varying the spot size (9 and 6 for reticular veins, 6 and 3 for telangiectasias) and fewer punctures and less glucose. For an incompetent great saphenous vein, she keeps laser with mandatory tumescence even when there is a fat pad, and suggests investigating fully laser-based treatment of tributaries to avoid the trauma of phlebectomy. She concludes that treatment requires caution, a combined approach, respect for the causal link between lipedema edema and venous symptoms, expectation alignment and more studies, acknowledging that much of what she proposes has a low level of evidence.

Lipedema and CompressionWhy compression acts on the mechanism of lipedema and not only on the symptom, and which modalities to use — stockings, leggings, pneumatic compression, micromassage and compression during exercise.
The lecture begins by recalling that lipedema is also a connective tissue disease, with fibrosis, chronic inflammation, capillary fragility, lymphatic involvement and muscle changes with sarcopenia, and it is from this triad that the role of compression derives. Four mechanisms of action are described: reduction of edema through the gradient, control of inflammation through mechanotransduction on adipocytes, micromassage — a principle included in the North American standard of care — and benefit during exercise. The speaker uses the giraffe as a teaching analogy: mean arterial pressure of 200 mmHg and venous pressure of 150 mmHg without developing varicose veins, thanks to an inelastic, collagen-rich skin that works as biological compression, a protection lacking in compliant human skin. He presents Karen Herbst's protocol published in 2025 with sequential pneumatic compression at 45 mmHg in sequential mode, one to two hours a day at home for 30 days, with reductions in total body water, extracellular and intracellular fluid and subcutaneous adipose tissue, a decrease in lymphedema risk on LDEX and symptom improvement as early as the second week — 83% improvement in walking difficulty, 73% in burning and 58% in pain sensitivity — as well as gains in almost all quality-of-life domains. He warns that incorrect use of pneumatic compression can cause fibrosis and recommends using it up to the waist, not just to the thigh. He also cites micromassage leggings worn three hours a day for four weeks, with a 7.1% reduction in leg volume and 81% in pain, and a meta-analysis of 12 studies showing accelerated muscle recovery. He closes with the practical prescription (stockings or leggings of 20 to 30 mmHg, micromassage, 10 to 30 mmHg during exercise, combination with infrared), with the caveat of choosing an adaptable product because the leg decreases in volume, and with a warning about devices that reach 260 mmHg, a pressure intolerable for the patient with lipedema.

The approach when the saphenous vein or a saphenous tributary is superficial and adherent to the skin is discussed: the recommendation is thermal ablation only when the vein is more than half a centimeter from the skin and, when it is adherent, eversion stripping with prior tumescence, avoiding residual phlebitis. The use of tirzepatide in a patient with normal BMI is debated, recalling that the medication is authorized for obesity, type 2 diabetes and sleep apnea, and that prescribing outside these indications transfers responsibility to the physician — the position defended is strict criteria and not yet prescribing in this profile. Another question concerns the correlation between bioimpedance and DEXA for diagnosis: the attempt to reproduce the Dietzel and Busso calculations with bioimpedance did not work, probably because of a short circuit when the thighs touch, and DEXA remains more accurate. Also discussed are photobiostimulation and laser in the inflamed patient, with no case series to support any effect, and the nodules seen on ultrasound in lipedema: the hypothesis that they represent hemorrhage, the increased resistance index suggesting inflammation, the halting of biopsies after six identical findings, the persistence of the nodule and the pain at follow-up and the future possibility of ultrasound-guided resection or liposuction. It closes with the question about the relationship between Dercum's disease and lipedema and about the role of compression in the already compensated patient, with the idea that the lymphatic system needs daily care and compression is one of the tools, especially useful at the beginning, when adherence to other habits is still low.

The speaker begins by distinguishing acute inflammation, necessary for survival, from the chronic low-grade inflammation that characterizes chronic non-communicable diseases, and shows that in lipedema tissue remodeling is gradual and continuous, with altered angiogenesis and a mononuclear cell infiltrate leading to fibrosis. She highlights a particularity: instead of a predominance of pro-inflammatory M1 macrophages, lipedema features alternative activation of M2 macrophages, with an apparently anti-inflammatory effect, keeping the tissue in a state of continuous attempted repair. She describes lipedema adipose tissue as dysfunctional and metabolically undernourished, which justifies strategies that increase nutrient bioavailability and gut modulation. She points to possible triggers — chronic LPS exposure, a diet rich in saturated fat, estrogen and hormonal changes, interleukins — and to activation of the classical and alternative complement pathways, raising the question of whether these patients might have some degree of immunosuppression. Microbiome: The Invisible Universe That Impacts LipedemaA lecture by a pathologist on the modern concept of meta-inflammation and on the microbiota, dysbiosis and fecal microbiome sequencing.
The speaker, a pathologist representing an integrative medicine laboratory, reconstructs the evolution of the concept of inflammation: from the classic signs of heat, redness, swelling and pain, through the loss of function added by Virchow, to the modern concept in which inflammatory molecules and cells are activated even without visible injury — what the cited authors call meta-inflammation. He argues that metabolic diseases, cancer, mental and intestinal diseases are the fruit of a seed formed by poor diet, inadequate sleep, environmental toxins, sedentary lifestyle, dysbiosis, trauma and toxic emotions, and that recognizing meta-inflammation enables prevention. He distinguishes microbiota, the set of microbes, from microbiome, the ecosystem formed by it and its environment, and describes the body's four microbiotas, the digestive tract's accounting for about 70% of the total. He reviews the count of cells and bacteria in the human body according to the review by Sender, Fuchs and Milo, with about 30 trillion cells — of which 25 trillion are anucleate red blood cells — and 38 trillion bacteria. He describes the oral microbiota, with 50 billion bacteria and plaque formation within six hours after eating, and the colonization of the newborn in vaginal delivery, with vaginal seeding as an alternative in cesarean delivery, noting that the microbiota stabilizes at three years of age and that after cesarean delivery this process would extend to seven. He lists the metabolic, barrier and immune functions of the microbiota and explains the stool–blood connection: in the leaky gut there is bacterial translocation, LPS activates TLR4 receptors and meta-inflammation sets in, with markers such as interleukin 6 and TNF-alpha, with repercussions on the gut–brain, gut–liver, pancreas and heart axes. Lipedema: How Pycnogenol Can HelpA satellite symposium on the clinical trial of Pycnogenol in lipedema, its metabolic mechanisms and associated uses in melasma and on long flights. The lecture was Dr. Breno's, who was unable to attend, and was presented by Daniel Benitti from his slides.
The speaker opens by explaining that he is standing in for Dr. Breno, the author of the lecture, in a satellite symposium. He presents a randomized double-blind trial carried out in Vitória with 100 women aged 18 to 40, with a loss of 7 participants and 93 adherent, half receiving Pycnogenol 50 mg every 8 hours and half placebo for 60 days. He characterizes the sample: mean weight around 80 kilograms, 73% physically inactive, only 33% on a diet and 31% with no reported family history; the main complaint was weight and the secondary one aesthetics, with only 2% reporting bruising. In the results, QuASiL fell from 88% to 63% in the intervention group, more than 20 points, while in the control group it even rose; body fat percentage fell from 39% to 38% and there was an improvement in self-esteem, attributed by the speaker to reduced neuroinflammation. The study's conclusion is a consistent reduction in QuASiL, with a probable effect on body fat percentage and self-esteem, and a need for more robust studies. He then presents an experimental study in rats fed a hypercaloric diet, in which Pycnogenol reduced fat gain, with 78% less visceral fat gain and smaller adipocytes, explaining the mechanism through AMPK activation, increased lipase sensitivity and a 58% to 62% reduction in PLIN1, as well as the PGC-1 alpha pathway and adipocyte browning. He suggests combining it with cinnamon — 500 mg of Ceylon cinnamon, with 23% less weight gain, lower leptin and triglycerides and improved microbiota — and with probiotics. He also addresses the use of Pycnogenol in melasma, a frequent complaint in these patients, with a dose of 100 mg per day reducing lesion extent, and the possible combination with Polypodium leucotomos 200 mg every 12 hours. He closes with the prevention of traveler's syndrome and thrombosis on flights: increasing risk above 4 hours, a 26% increase for every 2 additional hours, 4 times higher above 8 hours and 17 times higher above 12 hours, a hypoxic environment with 12% oxygen versus 20% in ambient air, and the regimen of 200 mg before a 6-hour flight and 100 mg the following day, together with compression stockings and hydration.

Nutrigenetic Testing: Your DNA Guides Your SupplementationA presentation of a commercial nutrigenetic test: what it analyzes, how it is performed and how it is read, with a basic review of DNA, genes and polymorphisms.
A continuation of the same speaker's previous lecture, now on the nutrigenetic test of the laboratory he represents. He describes a test that assesses the genome's response tendency in ten areas — metabolism, cardiovascular risk, aging, inflammation, food intolerance and gastrointestinal tract, vitamins and minerals, mental health, women's health, sports performance, skin health and men's health — based on 307 genetic variations, with saliva collection, DNA extraction and reading on an Illumina platform, delivered in a report of about 120 pages. He then gives a didactic review of concepts: the chromosome as coiled DNA, the gene as a stretch of DNA with a defined function, transcription and translation into protein, the composition of the nucleotide and what it means to sequence DNA, and defines a single nucleotide polymorphism as the substitution of one nitrogenous base for another. He shows how the report is read item by item, using metabolism as an example (energy expenditure, obesity, resistance to weight loss, response to weight-loss pens, non-alcoholic fatty liver disease, eating behavior, insulin resistance), with a scale in which the further to the right the marker, the greater that patient's difficulty in that item, followed by management guidance. He highlights as an advantage the fact that it is a definitive rather than circumstantial test, with the same result in the child and the adult. The final part moves away from genetics and advocates the six pillars of lifestyle medicine — nutrition, physical activity of 150 minutes per week, sleep of seven to nine hours, stress control with vagal stimulation, mental health and spirituality, and life purpose — as what would allow reaching 120 years of age with lucidity. He ends by citing other tests from the laboratory, including a test for early detection of colorectal cancer by methylation of three genes in stool, which would indicate colonoscopy when positive. The lecture does not specifically address lipedema; it is an institutional presentation of the test portfolio.

On the continuous use of tirzepatide at a dose of 1.5 mg after the condition is under control, the answer is that it depends on the clinical context and adherence to diet, that the studies reviewed do not support a continuous dose after improvement and that nothing needs to be forever. The speaker, a gynecologist at the Hospital das Clínicas of USP, separates absorbable implants (pellets, designed to last 5 to 6 months, which become incorporated into the subcutaneous tissue and are difficult to remove) from non-absorbable silastic implants (designed to last 12 months and removable at any time). He presents the studies he leads: CLARA, published in Menopause in December 2025, which showed linear, sustained and predictable pharmacokinetics of the 25 mg estradiol implant, reserved for patients who do not adapt to transdermal gel or do not reach the therapeutic estradiol level of 80 to 100 pg/mL; IMOX, multicenter and randomized, with a 200 mg oxandrolone implant in patients immobilized by anterior cruciate ligament injury; and STIM, with estradiol and testosterone versus placebo in women with natural menopause, premature ovarian insufficiency and surgical menopause, extended to two years and with thromboelastometry to assess thromboembolic potential. On gestrinone, he rejects the "beauty chip" label and recalls that it is a progestogen derived from 19-nortestosterone, with a pelvic antiestrogenic action that atrophies endometriosis foci, but with side effects that limit prolonged oral use; he warns that the implant does not always block ovulation, so the patient may become pregnant and a female fetus may be at risk of masculinization. He details the GLADE study, randomized, multicenter, double-blind and placebo-controlled, conducted at seven Brazilian centers, with 462 pre-enrolled and 100 randomized into two arms, all receiving a levonorgestrel IUD: there were no serious adverse events in either arm, and non-serious events were more frequent with gestrinone (44% versus 24%), including mild androgenization, hirsutism, acne and hair loss; the voice was recorded before, at three and at six months and assessed blindly, and only the software detected a difference of about half a tone, imperceptible to the patient. Rehabilitation in LipedemaThe role of physiotherapy in the rehabilitation of the lipedema patient in conservative treatment, in the operating room and postoperatively, far beyond lymphatic drainage.
The speaker defines rehabilitation as the set of interventions to restore functionality, autonomy and quality of life, and argues that physiotherapy in lipedema goes far beyond manual lymphatic drainage: it involves volume management, tissue rehabilitation in the face of fibrosis and inflammatory pain, musculoskeletal function (gait, postural correction, the calf and foot pump) and, above all, education for autonomy. She recalls that lipedema is not just accumulated fat — the patient has pain on light touch, leg heaviness, easy bruising, chronic fatigue and an orthopedic history with valgus knees, chondromalacia and collapse of the plantar arch — and that the intensity of symptoms does not necessarily track the stage. She advocates the physiotherapist's own assessment even when the patient arrives with a medical diagnosis, with history, physical examination and palpation, measurements, photographic records, thermography (with the caveat that it requires a controlled environment), a quality-of-life questionnaire and bioimpedance, and warns against ready-made protocols and cookie-cutter recipes. She describes complex decongestive physical therapy as the base already established in the consensus documents: manual lymphatic drainage — in which what matters is not the name of the technique, but knowing the lymphatic system, the direction, rhythm and gentle pressure of the maneuvers, since squeezing or rubbing collapses the system —, bandaging, individually prescribed compression (flat-knit or circular-knit, velcro wraps, considering symptoms, comorbidities, acceptance and climate) and exercise. She comments on pressotherapy, with a warning against devices bought without guidance for home use, and other resources such as photobiomodulation, shock waves, tecar therapy, electrical stimulation and magnetic stimulation, weighing that the choice depends on the professional's familiarity and on the risk of further inflaming the patient. She also shows the work in the operating room, with drainage, bandaging, taping, photobiomodulation and joint mobilization for early ambulation, and postoperatively, with pain relief, edema control, seroma prevention, early identification of complications and recovery of strength and gait, supported by a direct channel with the surgeon. She concludes that treating symptoms without restoring movement is temporary relief and that rehabilitating means making the result sustainable.

The first asks whether gestrinone causes weight gain: the answer is that it does not cause weight gain but does cause fluid retention — progestogens increase interstitial water, a phenomenon compared to the edema of pregnancy, which produces subcutaneous edema and may give the false impression of smoother skin, so that the supposed improvement in the orange-peel appearance may come from edema and not from the antiestrogenic action, something only a study measuring body water would resolve. It is asked why the implant and not other routes: the speaker denies being an advocate of the implant, cites other possibly better progestogens such as dienogest and drospirenone, criticizes the beauty-chip myth and says the worst scenario is the current one, with untrained professionals using poorly studied medication in patients without an indication. Adenomyosis associated with lipedema is discussed, described as a focus of endometrium in the myometrium, with pain and increased menstrual flow, in which well-indicated gestrinone improves quality of life. Mental Health and Psychoeducation in LipedemaA lecture advocating psychoeducation as the foundation of lipedema care, rather than an ancillary resource, comparing treatment dropout data before and after applying it in a Portuguese clinic. Two patient stories illustrate why available information does not, on its own, translate into adherence.
Valléria Roque, who works with psychoeducation in lipedema in Portugal and has the condition herself, opens with two cases: Mariana, 44 years old, 23 years without a diagnosis, type 3 lipedema, who received an individualized protocol, showed clinical evidence as early as the second session and still abandoned treatment; and Sandra, 66 years old, a nurse, with more than 15 years of the condition attributed only to obesity, who, after the disease was recognized, adhered to the protocol and remained under control. What separates the two, she argues, is not the diagnosis, but what comes after it. She presents a first study, from 2023, with a sample of 200 patients: 82 reported shame, guilt and low self-esteem, 89 had difficulty accepting the diagnosis, 69 had anxiety as a psychological impact, and in 105 lipedema affected their ability to relate to others. She then shows her clinic's own longitudinal clinical survey: in 2024, without psychoeducation, there were 322 new consultations, 280 active patients, 166 patients without treatment and 114 dropouts, that is, 41%; in 2025, with psychoeducation, there were 487 new consultations (a 51% increase), 414 active patients (a 47.8% increase), 341 retained and 73 dropouts, 17.66%. Her thesis is that the gap lies in communication: the patient arrives saturated with information and with little knowledge, without understanding why she wears compression stockings, what stabilizing lipedema means or what hypoxia is, and the high expectations she brings to the consultation collapse when none of this is translated into her world. Psychoeducation, she defines, is not positive thinking or a generic talk on passive acceptance, but a structured process, personalized knowledge, understanding in order to act, and health literacy, focused on the after — after the consultation, conservative treatment, surgery — because without understanding there is suffering. She also cites a preliminary estimate of about one million Portuguese people with features of lipedema, in a population of 12 million, and recalls the conditions that often accompany it, such as depression, chronic insomnia, anxiety, panic disorder, low libido and menopause. She concludes that psychological support is part of conservative treatment according to the international literature and that psychoeducation strengthens the patient in controlling the condition and reduces her emotional vulnerability.

Clinical Case Discussion — Conservative Treatment of Lipedema in PracticeA practical lecture on adherence to conservative lipedema treatment, in which the speaker shows the six-pillar method with scores and agreed goals that he uses in his practice and follows a patient's progress over four consultations.
Candemil starts from the problem he has faced since 2018: between the prescription and the result lies implementation, and it is the patient who implements — hence the high dropout rate when treatment arrives as a large block of subjective information. He proposes measuring in order to improve: he uses the QuASiL questionnaire, with 15 questions validated for Portuguese, completed before each consultation to gauge symptoms and inflammation, and creates a second score, which he calls PilarIL, dividing conservative treatment into six pillars — knowledge about the disease (psychoeducation), diet, water and bowel, physical exercise, physiotherapy and compression, medications and supplements, and sleep and mental health. Physician and patient together score each pillar, for a total of up to 60, identify which pillar is the 'first domino' to prioritize and turn vague advice into measurable goals for the next 30 days, reviewed at the following consultation with feedback through open-ended questions, without judgment. He illustrates with a 23-year-old patient, a medical student, diagnosed since 2024, who had worsened and gained 4 kg: she arrived with a QuASiL of 110, 77 kg and a BMI of 28, drinking less than 1 liter of water a day, with constipation, a sedentary lifestyle, irregular use of compression stockings, omega-3 and Pycnogenol, anxiety, poor sleep and binge eating. After 35 days the QuASiL fell to 83; at the following visits it reached 58 and then 38, while the PilarIL rose from 24 to 53 and weight fell from 77 to 65 kg in about six months, with symptoms such as cramps and itching eliminated. Over the course of follow-up, psychotherapy, a sleep diary, walks and weight training, a herbal medicine for anxiety, test-guided supplementation and tirzepatide were added. He notes that the PilarIL is not a scientific instrument but an office practice inspired by management — goals, feedback and Pareto —, and that the working hypothesis is that the higher the PilarIL, the lower the QuASiL.

Lipedema — Legal Aspects in HealthcareA medical law lecture on lipedema: how a lawsuit against a health plan's denial is built, and which precautions in medical records, consent, prescribing and advertising protect the physician treating the disease.
A lawyer specializing in medical law for more than 28 years, Gabrielle Brandão describes lipedema's passage from clinical invisibility to legal debate: a 2019 legislative proposal marked the beginning of litigation on the subject, followed by new bills in 2021, growing case law in 2022 and 2023 and the entry of Anvisa and the CFM into the discussion in 2024; today there is already favorable case law, that is, more cases granted than denied. She lists the most common health-plan denials — liposuction classified as cosmetic, compression therapy, drainage and physiotherapy, multidisciplinary follow-up with psychologist and nutritionist, compounded medications outside the coverage list, and imaging tests — and recalls that the agency's coverage list is not exhaustive, which opens room for legal action. She explains the practical path: the patient needs to request authorization even knowing it will be denied, because without the formal denial there is no way to file suit; to this is added a detailed medical report, with the ICD code (she notes that lipedema is in ICD-11, not yet in use in Brazil), robust documentation and proof that other therapies did not work, especially in requests for high-cost medication. On medical liability, she warns of the risk of confusing lipedema with obesity, of operating without a specific protocol and of promising results. She details what the free and informed consent form should contain — chronic nature and no definitive cure, possibility of progression even with treatment, goal of controlling symptoms, risks of the procedure, need for maintenance, alternatives and uncovered costs — and criticizes generic consent forms, inaccessible language and signature methods that do not prove the patient's authorship. She also addresses peptides and compounded medications, which may only be prescribed if cleared by Anvisa, the prohibition on steering patients to a pharmacy and joint liability among clinic, pharmacy and physician. The Bright Side of LipedemaClosing keynote that reverses the usual discourse about the disease and brings together findings showing biological advantages in lipedema — brown fat, muscle potential, fertility, breast milk and the baby's brain development.
Benitti opens with Roger Bannister and with a Stanford study in which merely telling someone whether or not they had the gene for running changed their measured fatigue and VO2, to argue that the patient's belief alters her own metabolism — and that telling her that her body is bad has a biological cost. He then reviews what lipedema studies show that is favorable and, according to him, usually ignored: very high expression of UCP1 and PGC-1 alpha, therefore more brown fat and more mitochondria, and a strong presence of PPAR gamma, which responds better to fat than to carbohydrate — an explanation he offers for the good response to the ketogenic diet and for the improvement of the microbiota. He cites a British study from March with a signature of women with lipedema showing low myostatin, which would indicate privileged muscle genetics, provided the fuel is protein and fat. He insists that lipedema tissue is not diseased but distinctive, with more than 3,400 different polymorphisms, and returns to the 2021 finding of a metabolism four times higher than that of women without lipedema, spent on detoxification. He compares the metabolic power of 100 grams of brown fat, which burns 400 calories a day, with one kilogram of muscle, which burns 13 to 15. He presents gluteofemoral fat as protective: a meta-analysis in the British Medical Journal with 72 studies and 2,528,000 people showing protection against all-cause mortality, as opposed to visceral fat. On the fertility axis, he cites a 1993 study in the British Medical Journal with 500 women in which each 0.1 increase in waist-to-hip ratio reduced the chance of pregnancy by 30%, and describes this fat as the body's largest store of DHA, reserved for the formation of the baby's brain in the third trimester and for breast milk, 80% of which would come from gluteofemoral fat, with the child's cognitive quotient higher the lower the waist-to-hip ratio. He adds the exclusively maternal transmission of mitochondria, a Cambridge study with more than 350 thousand people linking it to the child's height, disease risk and longevity, a study in which these women were 20% more accurate at reading intentions from the eyes, the 2019 Neurology comparison of MRI scans between women with the same body fat and different distribution, and the 2010 Dutch experiment with 19 people blind from birth who also chose a waist-to-hip ratio of 0.7. He concludes by asking that the patient get to know this biology and be proud of it, without promising that treatment will be easy.
